If you would like to surrender your animal to TLC Humane Society please complete our intake request form below and read the following information carefully.

INTAKE REQUEST FORM

Please complete the Intake Request Form to be added to our waiting list and someone will reach out to you if and when we have space to take in your animal(s). The timeframe for waiting is unknown and we are unable to give an estimate because surrender intakes are dependent on our adoptions, returns to TLC, and incoming animals from other shelters. If you have a litter of puppies or kittens please complete one form but let us know in the type of animal section that you are submitting a litter. For multiple animals not in a litter, you will need to complete the form below for each animal.

TLC requires all incoming animals be temperament tested at the shelter. Cats must be FIV/FeLV tested and dogs must be heartworm tested, TLC may or may not be able to take in animals who test positive for either of those diseases. All animals must also be vaccinated for rabies if they are old enough.

TLC is unable to take in stray or found animals, you must go through your local animal control for those types of intake requests.
